Authorisation to collect a parcel
So that a relative can collect your parcel or registered letter.
The template text in French (the one on the document) — « Procuration pour retirer un colis »
The document starts with « Je soussigné(e) [your name], born on…, residing at…, » (I, the undersigned)
autorise [Prénom NOM], titulaire de la pièce d'identité n° [numéro], à retirer en mon nom le colis ou le courrier recommandé n° [numéro d'envoi] auprès de [bureau de poste ou point relais].
Je joins une copie de ma pièce d'identité.
Translation (to understand)
authorise [First name SURNAME], holder of ID no. [number], to collect on my behalf the parcel or registered mail no. [tracking number] at [post office or pick-up point].
I attach a copy of my ID.
When to use it? You cannot go to the post office or pick-up point before the deadline.
Good to know: the person shows their ID, yours (or a copy) and the delivery notice. Some carriers have their own form.
