Assignment of a domain name
To transfer a domain name to another person or organisation, free of charge or for a price.
The template text in French (the one on the document) — « Cession d'un nom de domaine »
The document starts with « Je soussigné(e) [your name], born on…, residing at…, » (I, the undersigned)
titulaire du nom de domaine [nom de domaine], enregistré auprès de [bureau d'enregistrement], ci-après « le cédant », cède ce nom de domaine à [nom de la personne ou de la structure qui le reçoit], [numéro SIREN, SIRET ou RNA, ou date de naissance], [adresse de la personne ou de la structure qui le reçoit], ci-après « le cessionnaire ».
La cession est faite [prix : à titre gratuit / pour un prix de … €] et prend effet le [date de la cession].
Le cédant s'engage à faire le changement de titulaire auprès du bureau d'enregistrement et à remettre au cessionnaire le code de transfert s'il le demande. À compter de cette date, le cessionnaire est seul responsable du nom de domaine, de son utilisation et de son renouvellement.
Translation (to understand)

The assignment is made [price : free of charge / for a price of … €] and takes effect on [date of the assignment].
The assignor undertakes to change the holder with the registrar and to give the assignee the transfer code if requested. From that date, the assignee is solely responsible for the domain name, its use and its renewal.
When to use it? A provider or volunteer bought the domain in their own name and hands it over to the association or business; or you are selling a domain name.
Good to know: this is the document registrars (OVH, Gandi…) often ask for to change the holder. Both parties sign: on the same phone, tick “Two signatories”.
