To allow another person to pick up your child from school or a leisure centre.

The document starts with « Je soussigné(e) [your name], born on…, residing at…, » (I, the undersigned)
agissant en qualité de [père / mère / représentant légal] de l'enfant [Prénom NOM], scolarisé(e) à [école ou centre], autorise [Prénom NOM de la personne], [lien avec l'enfant], à venir chercher mon enfant [le date / du date au date].
Cette personne présentera une pièce d'identité.
Translation (to understand)
acting as [father / mother / legal guardian] of the child [First name SURNAME], attending [school or centre], authorise [First name SURNAME of the person], [relationship to the child], to pick up my child [on date / from date to date].
This person will show an ID.
When to use it? A grandparent, neighbour or friend picks up your child from school, nursery or leisure centre.
Good to know: the school may have its own form; also give a number where you can be reached.
