To lend your car and prove the driver uses it with your consent.

The document starts with « Je soussigné(e) [your name], born on…, residing at…, » (I, the undersigned)
propriétaire du véhicule [marque et modèle], immatriculé [immatriculation], autorise [Prénom NOM], titulaire du permis de conduire n° [numéro], à utiliser ce véhicule du [date] au [date].
Translation (to understand)
owner of the vehicle [make and model], registration [registration number], authorise [First name SURNAME], holder of driving licence no. [number], to use this vehicle from [date] to [date].
When to use it? Lending your car to a relative, especially for a trip abroad or a roadside check.
Good to know: check with your insurer that the driver is covered.
