Parental authorisation
To allow your minor child to take part in an outing, activity or trip.
The template text in French (the one on the document) — « Autorisation parentale »
The document starts with « Je soussigné(e) [your name], born on…, residing at…, » (I, the undersigned)
agissant en qualité de [père / mère / représentant légal] de l'enfant [Prénom NOM], né(e) le [date de naissance], autorise mon enfant à participer à [activité, sortie ou voyage] le [date], organisé(e) par [organisateur].
En cas d'urgence, je peux être joint(e) au [téléphone].
Translation (to understand)
acting as [father / mother / legal guardian] of the child [First name SURNAME], born on [date of birth], authorise my child to take part in [activity, outing or trip] on [date], organised by [organiser].
In case of emergency, I can be reached on [phone].
When to use it? School outing, sports camp, holiday camp, competition, club trip.
Good to know: for a minor to leave France without their parents, the official exit authorisation form (AST, on service-public.fr) is required.
